Chronic Disease Self-Management Education Webinars

Roughly forty percent (39.7%) of New York adults have a chronic disease such as arthritis, asthma, stroke, heart disease, diabetes, or cancer. Evidence-based community delivered interventions (EBIs), such as the Chronic Disease Self-Management Education (CDSME) suite of programs, are research-proven to help mitigate and lessen the burden of these conditions for the individuals and for the larger health/human services systems. NYSOFA supports the advancement of CDSMEs and other EBIs through its oversight and effective management of Older Americans Act Title IIID annual funding allocations to the AAAs and its successful efforts to secure numerous federal grant awards. Chronic Disease Self-Management Education: Why Be a Peer Leader?

This video provides information on the value of becoming a workshop facilitator — a "peer leader" — for interactive group-based health promotion programming known as Chronic Disease Self-Management Education. You'll hear from "Master Trainers" who provide the training required to become a peer leader, recently trained peer leaders, and from individuals who have been providing the program for many years.

Chronic Disease Self-Management Education: Information about the Six Week Workshop

This video provides information on the format and concepts presented in Chronic Disease Self-Management Education (CDSME) programs. These group-based programs meet for a set time each week for six weeks and can be provided remotely, in-person or by telephone. There are a variety of different program types that focus on various conditions (e.g., diabetes) or that were designed with cultural modifications. All programs are evidence-based, demonstrated by multiple research studies, and follow a curriculum developed by Stanford University to ensure required concepts are included and that all programs are provided consistently.

Exploring In-Person vs. Virtual Delivery of Evidence-Based Programs

As a result of the COVID-19 pandemic, many evidence-based programs are now able to be delivered through various modalities (in-person, telephonic, and virtual). In this webinar, participants will explore the delivery modes available for the Chronic Disease Self-Management Education (CDSME) suite of programs, learn to maximize recruitment efforts by delivery mode, determine where and how to focus recruitment efforts, understand how to review peer leader preparedness for the chosen delivery mode(s), and learn to determine your organization’s readiness for new delivery modes.

Chronic Disease Self-Management Education (CDSME) Program Tool Kit Delivery

The Chronic Disease Self-Management Suite of Programs includes an option to deliver programs via a mailed tool kit that can be completely self-directed and supported with weekly conference calls led by trained Peer Leaders. This is a great way to reach people who prefer not to utilize higher tech options like virtual sessions or for people who might find traveling to an in-person on site event difficult for any number of reasons. CDSME Delivery: Extending Beyond In-Person Programming

The advent of the COVID-19 pandemic created challenges to the in-person delivery of evidence-based programs like the Chronic Disease Self-Management Program. Developers and program delivery personnel have worked to fill this gap by expanding programming delivery modalities creating both opportunities and challenges. This interactive session will review and discuss best practices and lessons learned from this experience.   Presenters will consider the benefits and challenges of each delivery format, provide tips to enhance delivery in each format and consider the importance of program fidelity as we move ahead with this expansion.
