21-PI-10 New SHIP Unique Counseling ID Confidentiality Form
As of July 1, 2021, all Health Insurance Information Counseling and Assistance Program (HIICAP) counselors must update their SHIP Unique Counseling ID Confidentiality Form on an annual basis. To streamline this new policy directed by the Administration for Community Living (ACL), NYSOFA is requesting that all current counselors with an active SHIP Unique Counseling ID either fax or email the updated 2021 SHIP Unique Counseling ID form (Attachment 1) during the month of July. This directive includes SHIP Unique IDs issued within the last twelve months. For example, if a SHIP Unique Counseling ID was issued during the month of May for a new counselor, the counselor will be required to submit the new form during the month of July. This instruction will be required on an annual basis. Annual privacy review and training is also required (Attachment 2). Any older versions of the SHIP Unique Counseling ID Confidentiality form received after July 1, 2021, will not be accepted.

Customer Service Representative (CSR) is a system that is set up between the Centers for Medicare and Medicaid Services (CMS), 1-800-Medicare contractors and Medicare private insurance plans through a Unique ID system to allow State and local SHIP staff to discuss a beneficiary’s Medicare claim without the beneficiary being present.
