18-PI-11 NY Connects Satisfaction Survey

This correspondence serves to provide NY Connects with an Updated NY Connects Satisfaction Survey and companion Core Survey Question Set for local use. This survey now includes a question on overall satisfaction and experience utilizing the NY Connects No Wrong Door system for long term services and supports. As such, this survey (and core survey question set) will continue to serve as a required method to evaluate the effectiveness of the provision of Information and Assistance (I&A) to individuals contacting NY Connects
